how often should i increase my weight, and by how much? or is this determined by the user, and increased as soon as you notice gains/ease of reps?

strive to hit a certain amount of sets and reps with certain weight. when it becomes easier and u can hit x amount of sets and reps, increase it by a few lbs until u can get those same sets and reps, generally speaking.

on 5x5 /mehdi / stronglifts? till you do the same weight every set.
5,3,1 there's a formulated way to gradually increase.
on high rep sets, use a chart and opt to increase roughly 30% of those sets' in some weight or reps or both.

I've always liked to increase volume with X weight. Then reduce volume and increase X weight. Work back up to the previous volume with increased weight. Repeat.

so, lets say 10 reps at 100lbs and you get use to it, then increase reps for a little bit, then when use to that go back to 10 reps at higher weight.

this what you mean?

Well, I guess you were kind of on the right track.

Lets say:
Week 1: 3x3 225 lbs
Week 2: 4x3-4 225 lbs
Week 3: 5x3-4 225 lbs
Week 4: 4x4-5 - 225 lbs
Week 5: 5x5 - 225 lbs.
Week 6: 3x3 - 235 lbs

This is just a rough sketch, but I think it will give you an idea. You need to add volume rather than constantly ramping up in weight and testing your strength. Volume is used to build strength. You basically want to increase in cycles.
