Like the word implies, triggered a gun. Triggered is like pushing the right button (saying a certain thing) to cause a person to explode like a gun. Their bullet sails out in the form of a rant, argument, or rage.
Triggering somebody can be completely accidental or completely intentional (similar to trolling).
Sometimes it happens when somebody reads something they find extremely offensive, and they are so emotionally wound up in the moment that they have an uncontrollable response. They were triggered.
