I used to work for a pharmaceutical insurance provider, now defunct, called AdvancePCS. The worst day that I can remember there was when a woman called in because she had to get drugs for her son's cancer treatment and they were trying to charge her like $800.
Turns out that the last dose used up almost all of the Maximum Allowable Benefit for her insurance, which like $1200 a year. It was the second of the six doses he'd need to get treated.
