It's literal dick cheese

Forms in the foreskin, if you don't clean it properly, if I remember correctly ; I have a vagina so I'm not way positive

How about smegma in the vagina?
I've seen stuff that looked like smegma, at least. Kind of ruins the experience of giving head.

I would assume that someone with poor hygiene could obtain something like smegma in the vagina
