Belt + Armor are definitely best from the list and are used in Gm Melee duels. Belt is for Barbarian whirlwind build as vasya said and armor can be used on all melee characters.

I would make this, just my opinion so nothing sticky ^^

1. Start 50fg (ft can be close to start maybe ^^)

2. Start 100fg (ft can be 500-1000+ maybe more maybe less im not barb player anymore so ask barbarians friend :) )
All i can say is that 40str requirements is very goood
30 Strength is Max add (24fhr is max add)
And 285 Defense is Perfect Roll for 197% Ed on this item :)

3. and 5. as i said i dont see any usage on those boots.
