yeah so ambush wiped me and my entire group out instantly so i shrined and figured we get a new maze or something, nope ambush is still there, died twice in like 10 seconds

They don't die randomly, and can only die in Catacombs. No other section of the code will spawn a death. Nice try though. Sorry, I will not reply further to HC related deaths.

Just checked the logs, you were in catacombs. If you want to waste my time further, you will not like the outcome.

You were in a fight, in catacombs. Your group was engaged. There is no mystery, other than you not understanding how LS is played with the new version. Make a SC character and see for yourself how it works.
