Correct, the only mod that is usable in the ACC slot is Enhanced Effect, which is why it's typically used for a heal charm.

Check this out. It depends on what weapon you are using. Also, there is really no need to stat more than 5 into vita in softcore.

Correct (but usually you just max dex because of your base strength being above 25 from the git go)
