It's my experience that the trick to making these work comprise of two things:

1) It's best if there is an actual name (or at least a noun) involved. Hot Carl. Dirty Sanchez.
2) If that doesnt work, and adjective and a noun seem right. Frothy Walrus

When all else fails, two funny words together work. Donkey Punch.

I guess the most important thing is to make people ask questions. "What's a Cleveland Steamer?"
