I have had #2 happen to me so many times and scare the crap outa me... I would check all your characters via your profile list, not by actually logging in, sometimes the in-game inventory is funny and likes to hide stuff on me, especially when dealing with over your max Equipment Slots (after reset)

did both

didnt find anything

they should be on that char together with my other heals:

Never upgraded my vault again after :p ( probably the increase in monthly fee also did its part ;d )
