You buy the in game gold then gamble at one of the NPCs and most likely don't get anything, but you never know

It's one of the most efficient ways to get the best +2 amulets that are typically only available from Hell Diablo:

At level 86, 1/10 of your amulets will be lvl 90
At level 87, 1/5 of your amulets will be lvl 90+
At level 88, 3/10 of your amulets will be lvl 90+
At level 89, 2/5 of your amulets will be lvl 90+
At level 90, 1/2 of your amulets will be lvl 90+
At level 91, 3/5 of your amulets will be lvl 90+
At level 92, 7/10 of your amulets will be lvl 90+
At level 93, 4/5 of your amulets will be lvl 90+
At level 94, 9/10 of your amulets will be lvl 90+
At level 95+, 100% of your amulets will be lvl 90+
