Hello, so there is ladder and non ladder on D2R now so you would have to chose which one you are willing to play, keep in mind Blizzard said ladder would reset every 4 months in D2R and if that is true we should expect a reset near the end of this month. As far as regions there are 34 I believe. US, EU, ASIA. You cannot play cross region BUT you can change your region from one to another to play in that region if you have a buddy there or need to do a trade in that region.
