Only Triumphant items return mana when using Elemental attacks and Bone Spells, according to the Arreat Summit. These items are a specific type of magic suffix that grants mana leech when dealing damage with elemental attacks or bone spells.

/e The steal comes from how much damage you do to the monster. So 5% Mana Stolen will return 5% of the damage you do to a monster. Life and Mana steal only work on Melee, and physical ranged attacks, only Triumphant Items will return mana when using Elemental attacks and Bone Spells.

If you have triumphant prefix on something (+1 mana per kill) and say use nova and flatten a mob of 10 you will get back 10 mana
