Try This:

Windows Vista and 7 have known compatibility issues with Diablo II/LoD ... to fix them, take the following steps:

Right-click your Game.exe file and in the compatibility view, make certain you are running the game under Windows XP SP2/3 mode, and DISABLE DESKTOP COMPOSITION. This should allow your game to start normally, but you will probably have serious lag spike issues that need to be resolved.

On your Desktop, hit Control+Alt+Delete and open your Task Manager program. Select the "Services" tab, and click the "services" button at the bottom of that screen to access your services manager. (you can close your Task Manager now) Scroll down the list until you find "Windows Media Player Network Sharing Service". Stop this service, then right-click it and select "properties" and then DISABLE this service.

This will fix your lag issues and compatibility issues with D2 in Windows Vista (6.0+) and Windows 7 (currently release candidate version 7.1.00)

i guess u dont have any FPS issues with any other games?

then google for svens glidewrapper.
i suggest maybe putting a fps limit of 6xfps instead of using vsynch (expierienced some problems with my ATI using vsynch).

If you are getting low FPS in all games you play, it sounds like you need to update your drivers.

Hopefully this step by step guide will help if you do. I believe this should work with Windows XP, Vista and Windows 7

1) Click Start

2) Click Settings

3) Click Control Panel

4) Click "Device Manager"

5) Double click or hit the drop down menu for "Display Adapters"

6) Right click on whatever your graphics card is that is listed and click "Update Driver Software"

An example of what might be there would be mine: "ATI Raedon HD 6850 Series"

first thing that comes to my mind then wud be trying another graphic card.
coz if u still have the issue it would almost be guaranteed that the graphic card isnt the problem then (and thats where i wud probably just reinstall my system; maybe try a restore point first).

how regular do those lags occur? every 10sec or every 5min? how long do they last? can u memorize any actions / installations / anything that took place between the date where ur system worked normally and now?
is the display quality the same as before for all your applications and it just gets slowed down? are u experiencing any lags browsing windows explorer?
how about data transfer? slower than before? how about youtube videos (do they sometimes lag as well even when fully loaded before)?
if u alt+tab from a 3d application lag is gone instanly?
what is your CPU / GPU / HDD / Memory temperature when u expierience the lags?
how many / which fans cool down your system?
are you sure your system is clean (scan with MS security essentials + malwarebytes antimalware or smth of that sort)? its pretty unlikely an infection cud be the reason, unless it randomly screwed with some of your files in the windows directory (drivers/dlls), which doesnt rly happen alot afaik (but its always nice to have a clean system ofc).

your current driver is:

GeForce 280.26 Driver
released: 2011.08.09 ?

if you are unlucky that bundle driver just doesnt work well with the low end of the spectrum (your graca) in which case it wud even make sense trying previous versions, but usually u would know if/how it works after a driver update so thats kinda unrealistic.
