Do you still have all your items on PS?

And when you log on into battle.net / Accounts / connected accounts - you can see your actual PS-nick under playstation network?

Your problem is that you did not use the same Battle.net tag / account on PC as you did on Playstation, you need to use the same Battle.net account/tag for cross-progression.

Yep, to carry over progress from Playstation to PC you need to log on and link the Battle.net accounr/tag you used on Playstation over to PC when you install D2R instead of using your old Battle.net PC account.
