Weapon --- Black runeword in a flail (Of all the clubs/maces, only flail-alike can have 3 sockets at -10 base attack speed, so this is what we'll use. Do not take exceptional/elite versions because smite damage depends on the shield, not the weapon. Plus it messes up your style...)
Armor --- Smoke runeword in any elite, low-str armor (Try getting dusk shroud, although anything up to great hauberk will do just fine. Avoid medium and heavy armors)
Helm --- Guillaume's face (Set helm from the Orphan's call set. This really outclasses anything else you can find. It's also very, very easy to find)
Shield --- 4 x pdiamonded paladin shield of any type (I hope vortex didn't even cross your mind. High str requirement not being the main issue here, but the block! Vortex has shitty block at so high str requirement that it just insults me
Avoid targes of any kind except sacred due to low block, and normal shields in general due to low defense. Try finding something with about 30 to base resists. Anything more is just too valuable to spend on a build like this)
Ring 1 --- Raven (This is a must because of cannot be frozen. They are everywhere though)
Ring 2 --- Nn
(Use anything you want. On my smiter I didn't have it at all)
Belt --- Crafted blood belt (Just a source of open wounds, can't be crafted from any belt though)
Boots --- Goblin Toe (Dirt cheap)
Amulet --- Cat's Eye (Crap amulet and all are aware of that. Should be much cheaper than the Highlords at the start of the ladder, so this should be your #1 choice. If you get the needed 20 ias from gloves, you can use pretty much anything here)
Gloves --- Not needed (You got everything you wanted. In case you don't have a 20 ias amulet like the cat's eye, use Laying of hands or even Sanders gloves here)
Switch --- Wand with charges of Life Tap and a high block shield (Buy the wand somewhere, they are all over the place. You need a high block shield because sometimes you use the wand in very unusual situations, and the block is what will save your ass)
Prebuff --- Treachery (In anything. If you can't get the lem rune...well, try at least)
