When items list a Level Requirement, the number listed is 75% of the highest prefix or suffix level (as shown on this page) on the item. For example, a weapon with a Meteoric(27) as the highest prefix (or suffix) would have a level requirement of 20. Rare Items do not list the magic prefixes and suffixes by name so you will have to figure out based on their stats what magic prefixes and suffixes are being used.
The highest prefix or suffix level is the +2 skill levels, which is :
Priest's +2 to Paladin Skill Levels Paladin Shields(50), Scepters(50), Hammers(65), Maces(65), Shields(65), Swords(65), Amulets(90), Circlets(90)
90 * .75 = 67.5 which means level 67 req no matter what the fuck the item is.
Transmute Formula 1 Perfect Skull + 1 Rare Item + Stone of Jordan = 1 High Quality New Rare Item of the same type
This formula has the possibility of creating a higher quality Rare Item than the following formula. ilvl = int(.66 * clvl) + int(.66 * ilvl) That is, the ilvl will be equal to .66 times your clvl (rounded down) plus .66 times the input item's ilvl (also rounded down).
Transmute Formula 6 Perfect Skulls + 1 Rare Item = 1 Random Low Quality Rare Item of the same type
You can use this formula to get another chance at a Rare Item of the same type. ilvl = int(.4 * clvl) + int(.4 * ilvl) Player level matters. This formula will not work on items larger than 3x2.

Circlet Clvl Req: 52
Item Level: 70
Diadem Clvl Req: 64
Item Level: 85
The item level is used with your character's level to determine what can roll on the item. That's why you use high level characters like 90+ so you automatically get +67 or more ilvl. Using pskulls ruins it because you get +36 or more but the item level at 85 gives you 32, 36+32 can't get you to 90.
The numbers in brackets listed next to the items the prefixes appear on are the affix levels. They are used in several calculations (Gambling, Horadric Cube Recipes, Crafting, more). These are not to be confused with the Level Requirements that you actually see listed on items. These levels are also not the character levels required to use or find these prefixes.A level of 30 on a prefix doesn't mean you have to be 30 to see or use it. However, if your character level is at that level shown next to the item, you will be able to create (Horadric Cube) and Gamble these magical bonuses.The uses of these levels will be shown in more detail in other sections of this guide.

I agree that import versus legit doesn't matter cuz as I have proved, you can't tell the difference and obviously people love godly items. This is the same for legit versus duped. no one can trust if your shit is duped or legit, but we just hope it doesn't poof. Similarly, we can't tell if it's import or legit but we just hope it doesn't poof.
