the amulet you search is impossible. cannot have 15+fcr str dex life
also 20 dex is highest, so 20+ is abit silly.

edit#2: wow retards, for 15+fcr you need to have 10%fcr (one affix), x dex (one affix) x str (one affix) x life (one affix) and one x skill (one affix) = 5, impossible on a craft duuuhhh. 4 is max.
best for you = 2pala 15fcr 20dex 60life or 2pala 15fcr 30str 60life or 2pala 15fcr 20dex 30str
