can sumone tell me how to get hell difficulty

and also what are cryptex and other quest items used for that just keep piling up and using space in my inventory?

Hell - join a game with hell and keep it.
Cryptex - not used till tokyo (unreleased)

They had it from early on when it was still available to play and never changed the difficulty.
