to start off, i dont know if this is something new or not, but i found it out by myself.
it's a very simple trick:
1- find a quest that allows you to pick 2 items as rewards.
2- complete the quest and go to the npc, in order to get your rewards
3- click on the npc and, once on the reward choosing screen, pick ONLY ONE reward, close the window
4- repeat step 3 until there are no more rewards available
(i dont know if by leaving the game before picking the last item you can get all the rewards again, cause i cant try anything in this old pc i'm using now)
also, keep in mind you can identify the rewards before choosing them
