Can't remember, I just got it as I played through the game.

There's a secret door in one of the pthumeru dungeons and behind that door, there's a bath messenger that sells it.
I think bloodborne wiki has everything you need to know.

I got mine without buying it but by just playing through the dungeons. Are there other ways to get it?

Just killed Rom again in the chalice dungeon. Think I died 2 times due to sheer tardness, got enough gear/level to bruteforce most bosses it seems, so I got sloppy and just died to a swarm of spodermen

Hmm. I wonder where mine dropped or was found. I know for sure that I haven't seen any vendors in the dungeons yet.. I'm almost positive.

I just killed the last boss on floor 4 (blood-beast thingy after rom) and the defiled chalice dropped for me.

There're these secret doors that look like a wall, just gotta hit 'em once and they slowly open or become visible. They look pretty obvious tbh.
