AV and HDMI i'm guessing.

i doubt that
that would lower the quality on one of the tvs

and i doubt ps4 will have 3hdmi inputs

I've seen a triple screen for DriveClub. I know for a fact PS4 is triple screen. HDMI splitter maybe

Depends on the ports it has and can actually support multiple displays.

Graphics card. Must have at least 3 separate ports like DVI-I, DVI-D, and Displayport. Or probably swap out the DVI-I port for an HDMI port.
