I've tried about ten times to compile this form in every field, gaining only an error message with unreadable scripts (I've not Korean language installed on my WinXP for now).

Now, i'll try to explain how I filled every field, and I'll be very pleased if you can tell me what I've done wrong, or even if you can tell me a way to compile it correctly.

Then, another thing, I can't understand what are that 4 choices in the lower part of the form...I can't understand what must go in there.
