how are people finding these?

people constantly have them, people I know find them or whatever etc
how do you find them?
nightmare survival makes sense but...that doesn't explain insane guardians and other fixed rewards

This. Generally speaking (after completing a map) you get better "luck" with the Tavern if you did not host the game.

If you are not "finding" any decent pets, it's because you probably always host your own games.

not true i host all the time with my friend and most of the time hes getting better pets than me from tavern. on wave 19 we got 140+ gaurdians i cant even imagine what it wouldve been like if we completed the whole thing and got the same luck.
