There are cheaters etc, but you dont have to play with them. Also, VAC started banning a LOT of them last week. We shall see how things are from now on.

Tbh idk what you mean by demon. And I have been looking at the DDOF for sometime now, I just like to ask my fellow Jspers for their opinions as well

Yeah, it's a fun game. I strongly suggest you do NOT get powerleveled on your first character and work through each of the campaign maps either on your own or with other non-lvl 74 players. The first 100 hours of this game is by far the most fun. I still remember the first time I beat the boss on Summit map on HARD with 3 other people. Most epic fight ever. Took over an hour, lol.

As for the grinding, well it's up to you. You need fairly minimal gear to get through the campaigns and most challenges even on the hardest mode, and even less than that if you play co-op. After that it's either grind (in survival maps) to 2k-2.5k+stats or quit since there's nothing else to do.
