I would guess that you don't experience repetitive stress injuries. The main problem is doing the same motion over and over. On a keyboard-mouse combo, I can rotate which finger hits the key button, and move my hand into different positions constantly. With a mouse click, my hand is in the same position always (think that slightly curved cupped position of a hand over a mouse). And, it is very strange to try to use a different finger other than the right pointer finger, which is the one that very quickly starts giving me trouble. As soon as it starts hurting, it's all over.

And yes, I don't plan to do anything differently until I get some reasonable sense of confirmation. I would assume that something like an AHK (which would bind like, the fish button to one keyboard key, and the snag button to another) would be unacceptable. Though, being able to use just a keyboard and not have my hand in the mouse-hold position would be additionally helpful.
But, I'm holding out hope that using the built-in Windows 10 functions to bind the left mouse click to the numpad 5 (in the accessibility settings) would be permissible.
