It appears randomly on a tile you explore in the catacombs.

If you do full explores maps can drop with guaranteed wells.

This is a lie. Maps only drop wells if is a feature of the map you used.

Regular catacomb runs (not group whistles) have about a In 5 chance of dropping a well. (Depending on your luck and the lvl catacomb you use can make this rate better but in my experience this is my average).

Angelic cured mountain map
Level Req: 5
Use Item to Receive Effect

+1 Warm Lights
+1 Evil Presences
+5 Treasure Chests
Quantity: 3

I personally don’t have any maps with wells so if someone wants to drop an example here would be great
