I played LS last time I think around 8 years ago. Getting back to it as it's a perfect chill game for when I'm bored at work and home.

I've been reading some guides etc. to help me start, but I have one simple question I'm very curious about.

DO YOU EVER LOSE YOUR ITEMS?

If so - when?
I've seen that when the ladder reset happens you get to keep your loot but lose your exp. So... Does it mean people have their gear for years, or am I missing something?

yes some people have had their items for many years. When the ladder resets you keep all your items but all your characters start anew at a feeder class

Nope. Once you do the master quest. The items are returned to your inventory. If someone resets, same thing. You never lose your items unless you delete/shrine them, or you put them up on the marketplace and leave them there they are deleted after some time.

Seems weird, it seems like it's difficult to find something worth anything as players have had their useful loot for so long

It should be noted that YES you can lose your items accidentally if you play on the phone and accidentally shrine or delete your items.

Most players, including myself, suffer from HOARDING! We just don’t want to let go of stuff......

There were years between updates where drop rates of good items were terrible.

Even with drop rates improved now, excellent drops are still super rare.

Even people with great items are always looking for upgrades.

There are also lots of former players with amazing gear who haven't signed on in years

Can not necessarily will. There have been people who have had things in their vault for many years without touching them and didn't lose a thing. It's not a good idea to leave things in an expired vault for long but nothing seems to happen regarding this.
