1. Yes. Number of items you can throw in is random. anywhere from 1-150 items. 90% of them are 1-15 items
2. 1 in 100 to 500 squared of the catacombs. I usually hit 4 from level 30 to 71 without additional well event.
3. 15s by muters needing level 15 items. Otherwise 20+
4. Fight well mobs with similar items level and char level. I will do level 15-25 items with a level 20-25 char. Level 45-55 items with a level 55 char. Fighting a level 20 mob at level 70 will give very few side drops.
5. Yes it used to be in town.

Only way to get overspawn items is from ML.
The item you throw in is the first item out and the mobs stats are based on that item. So if you throw in a level 55 fire and it comes out 250ee. The mobs casting attacks will hit very hard with the element fire.
So from that we've theorised the first items stats should already be set when you throw the item in.
The side drops however are from how much ML was used when the mob died.
This was tested using 0 ML on throwing item in and killed the boss with 35 ML and only got overspawns on side drops.
So the general strategy is to throw the item in with as much ML as possible, then kill it with as much ML as you can.
Personally I usually use melee class, so will throw the item in with ML wep/charm/armor. Then swap to ee weapon and kill with that gear. If its a particularly tough mob then change armor too
