Im not exactly sure what your saying/asking

yes all items eventually disappear, sometimes items disappear almost instantly but not real common.

It takes about second to appear in your inventory if you are purely soloing.

There is always a chance to lag and rush to town and the item doesnt go with you.

are you trying to self mule?
I would understand if english isn't your first language. Google translate from your first language to english exists.

my item drop column is open with displayed items in the catacombs. if I keep walking to attack the next enemy poofs sometimes items without having picked them up they are just gone

German explanation for you:

Wenn du das monster killst und danach nichts tust bleibt das Item - Sobald du wieder auf whistle oder engage drückst bekommt das item einen unsichtbaren Timer (glaube ca. 5 Sekunden) dann verschwinden die Items. Ansonsten wäre es unmöglich einen ganzen Climb zu spielen, weil in der Dropbox tausende von items wären.
