If you're hexed, this is a good set up(add a bneck on the end if you're 120, change t10 stuff like prom and tyranno for t11):
It's generally not worth going swapping out set ups but if the map is coming to an end and you happen to die at the start/right before boss you can swap out to second or third loadout as appropriate.
The third loadout is no big deal, really. You can use it for Hope-devourer and Dread if you want but it really doesn't matter. Like Sir Witch-King Obtaurian said, though, use your common sense. Don't bind GPB because GAUNTS ARE BETTER. I guess maul and rapier would be a fun 5th bind in some circles, though.
The ranged load out is nice for flesh/necro etc. The prim 2h is in the ranged set up for any doors left. It'd be more than retarded to go in to a GD without a primary weapon.
