So my suggestion is quite simple.

Problem:
Every time you wish to Block This User or Add to address book someone from your PM, it doesn't give you the chance to add a comment.
You'll need to visit your blocked users/address book to place a comment.

Solution:
Every time you click on Block This User / Add to address book it will place a box like the following so you can place a comment with the user:
(the username should be already typed in when you click on them)

Really good idea.
This would make it a lot easier and I wouldn't forget why I blocked someone
