Avatar or not, you haven't indicated that they can contact you unexpectedly - this would mean the blocking feature is working as intended in my opinion.

graphic theft accusation thread if one has evidence of a violation of that nature as well (this is more regarding your reference to the same avatar used by a few people).
Some people on a block list can Trigger individuals. Some people have "photographic memory" and just see no sense in seeing something they don't want to see when using the blocked feature.
No use in mentioned a user can be contacted unexpectedly, as it's well determined responding to spam on a trade forum can result in actions taken against an account. This feature is not about the other individual still being able to engage in sub-forums or other parts of the site but rather let you, others or myself see less of them when managing the blocked list.
As for avatars, it is nothing to do about reporting avatars. It is more of the sense that a not everyone is a flock is the same and if someone is on a blocked list while sharing avatars or avatar themes then the user blocking them may not want to associate or speak to others alike, regardless for all the variety of reasons why or when.
This feature is just a way to calm a small section of the site but has complexity within it. I may have mentioned it already but maybe it should just be an option for that section of the site and not a complete change, much like we have he option to see avatars and signatures across the whole forum.
