In my university program, i need to make a personal statement for my field placement in the next term. I never wrote one before, and i need to hand it in with my other registration forms. I was wondering if i should put anything in the header? or do i need to put my contact information on the statement even if they have all that information in my registration papers? or should i just write my one page statement with nothing else on the page and hand it in with my other papers? Sorry if it is a stupid question... they did not give us any more information
