Africa, I believe.

They burned down forests and used the ash as fertilizer.

Also South America, to the detriment of the rain forests.

Just wondering, how could you not find this? If you typed "slash and burn " into google with auto-suggest on, the 1st option is "slash and burn agriculture" with the first 5 results or so offering comprehensive histories of worldwide implementation of this method.
