Completely depends on the image + the background

Background Eraser
Magic Lasso Tool
Pen Tool

Use a mixture of all 3.

pentool in illustrator is the easiest for me... someone once showed me how to pen tool in photoshop without the fill showing and I still know how but its just not as easy to me.

If you're bad with the pen tool you could always use the circular magic wand (I don't know what its real name is) on photoshop cs3 if you have it.
