check if youre currently using a tool
if youre resizing it (CTRL+T) it will show
open recent
close
close and go to bridge
save
save as
check in
but itll fade out save for web, so the marquee tool that youre using/last used might be the problem
btw whats up with the shortcut name for save for web :s?
normally its 'just' alt+shift+ctrl+s

Yeh, save the file as a .psd, then re-open again, and try again. Make sure you have no selections made or resizing or anything, then retry

ok i talked to a mate about it and heres what i pretty much got out of it
if this problem only happened recently then ignore the following
go to Photoshop >help> about plugins and scroll down to see if its there
if its not there do this
go to photoshop>plugins>import/export and see if theres a file named save for web
if its not there pop in your cd and choose custom install > only plugins or something like that > and then only choose 'export'
if its still not working check if the image has 8bits each channel and it isnt on multi-channel
go to image>mode>choose mode and uncheck multi-channel if its selected, mine's on rbg color but im using windows not quite sure if its any different from a mac though
and this is the last thing he told me you could do,
exit photoshop, go to your user profile > application data > adobe > save for web folder > 1 folder of the version number should appear, choose that one > there should be a file in there called save for web preferences, delete it and empty ur bin(dont worry itll create another one of those files automatically) then restart photoshop
if any of that doesnt work out, i think you might be screwed :<
oh and a last thing, check if there are any updates, photoshop > help > updates, if its grayed out it should be up to date
