Google every superunique location in the game and check all locations

if you were at throne it should have spawned there on a wave boss, or at baal chamber if all waves were already wipped out

Dude ... the same shit happened to me yesterday and now AGAIN! Waited in Act 5 town in a freshly created game as always when I hunt Diablo and he didn't spawn ... trying to find him in the entire game now ffs wtf is that?!

just fyi it didn't spawn at baal wave. checked a bunch of uniques and no clone so wtf
