Need open wounds if you have low damage, as it prevents monster heal on it's own, you don't need lt wand if you don't mind juving a lot.
But you can shop lt wand super easy.
Even a minimum source of cb is enough, more is just a qol - guillaume+gore is enough as cb+ow source Even gore is enough, but it will take ages I suppose

prevent monster heal no, but open wounds yes. It is the only one that works with ubers. I think poison also works, but I'm not sure.
Dracul's grasp is really handy because it has open wounds and chance of casting life tap, but the wand also works. Just gotta be fast or tanky to not get one-hit killed by uber meph with his charged bolts.
I have a hammerdin with one hard point in smite and I can breeze through ubers with 50% CB (35% from guillaume's face and 15% from gore rider); I just equip a grief (got super lucky with a Lo drop when leveling him in act3 spider forest) and a Kurast shield with resistance and socketed with 4 perf Diamonds

You need 20% crushing blow, prevent monster heal and a life tap wand.
I killed clone with fleshrender and full rare/shopped items and it was easy as fuck. My full build is worth 5 Fgs max and I did not drink a single pot.

This guy did it with a level 18 paladin, you will be fine with super budget stuff.

Only smite. 20 smite, 20 holy shield, 20 fana makes it easier but I'm sure you can do it with 1 point each if you don't want to respec.

Otherwise plenty of People offer dc kill service, myself included. I don't charge anything.

I would add that if you don't have a shield with lots of resistances stacked to counter Mephisto's Conviction, putting one point into Salvation may be helpful against him.
My hammerdin was at lvl 76 when I did the first uber run and got one-hit killed by Mephisto like 6 or 7 times before getting the lifetap working from Draculs. But after getting Salvation, I never got killed by him again in the other runs I did
