Simple answer: Server-client desync.

It's the same reason I die with my invulnerability cooldown used, when it should have saved me.

You have to be pre-emptive when it comes to life saving cooldowns.

you can dodge it 3 seconds in advance, but if the server decides it doesnt like you; youre dead.
