AV was one of the greatest challenges in all of the final fantasy games until they decided to give everyone a break :(

They shouldnt have given the break <_< . Games need the truly challenging fights that you need to revisit a few times and take quite a bit of grueling/grinding/building up to the moment where you get the chance at the fight. 
Beating AV used to be a true accomplishment... oh well :p



AV for sure. I was part of an LS on Leviathan way back that attempted AV a hundred times, and got very close a few (after some things were discovered). One thing goes wrong though, and shit hits the fan. Definitely the hardest just because of the amount of teamwork, timing, and gear it takes... that is, before the nerf/cap increase.

The dudes that went to the hospital was for pandemonium warden.. He wasn't hard, but just very very long.. And this story (about the 18hour fight and guys giving up due to exhaustion) actually caused SE to edit the Warden so they didnt get sued.

Excluding FFXI I would have to say Major Numerus from FFX-2 International + Last mission or Chac from FFX-2 without triggering oversoul (nor cheesing it with cat nip).
