The key is understanding that XXX has a 0% chance and understanding why XXY has a 25% chance of occurring.

The chance of the first two missiles being XX is 25% (50% * 50%), and then the third missile always hits face in that scenario so its 50% * 50% * 100% = 25% chance of XXY occurring.

100% chance if it's your minion
0% chance if it's the enemy minion unless you're at 1 life with no cards in hand

Yeah no.

Having to win multiple 50/50s makes the chances less than 50%.
