but from what i see through playing a long time, and i remember chatting with grunt about this a year ago, the biggest contributing factor is POSITIONING
ill just quote a friend since i don't have much credibility - this comes from AzN.Ezzy about positioning - "positioning is 70% of the game"
a good player knows how to position himself - example is when ur facing a trilane, you're never caught off guard and you always keep your range
facing a cm wd kunkka trilane, as likely as it sounds, will probably rake up kills in many scenarios (ignoring strategic picks such as putting es in that trilane)
sometimes you will see a hero get away with like 10 hp after eating a tango in the middle of a gank by those heroes, that's not luck that's skill and positioning
players i listed above (including the humble tsumugi) have a good general understanding of that concept from what i saw - maybe not all at the same level, but there ya go
e/ i forgot about phox i dunno if hes still alive but iirc he should be pretty good still
