Going to use this thread to list all the bugged quests I come across, keep the discussion to a minimum unless you have a fix for a listed bug.
The ones I'm posting will be for the PC.

Quest: Shielded Favors
Location: Southern Shelf
Quest giver: Sir Hammerlock.

If you shoot the fuse box to go past the electric fence prior to getting the quest (say you want to do some random exploring) the quest will bug out, you will be required to save and exit and start again. The quest requires you to first run through the fence and then pick up the fuse. With the fusebox destroyed prior to getting the quest, the fuse cannot be picked up. Quick and easy fix but I'm sure it annoyed someone already.

Quest: Do No Harm
Location: Sanctuary
Quest giver: Dr. Zed

This quest has you chop open his patient to get some eridium. After getting it the quest has you go to Tannis to talk to her. After talking to Tannis this quest can go into a stasis, to where it shows you've talked to Tannis, but won't let you progress. I believe this is caused by opening the vault in Roland's house before "officially" meeting Tannis.

Solution: If you get this bug QUIT WITHOUT SAVING IMMEDIATELY. Hopefully you didn't hit an autosave. The problem is Tannis has an intro, and something causes that intro to not happen. If it doesn't, you will get this bug.
I reloaded my last autosave. grabbed the quest "Do No Harm" got the eridium. WAIT UNTIL Dr. Zed is finished talking before going to see Tannis. Go into the house facing her and strafe for a moment, the intro should happen. Then talk to her and complete the quest, then continue with the quest to open Roland's Vault.

Hopefully this made sense, and works for everyone.

Slot machines are giving me low level items and the cost is well below where it should be for my level in games that I host. No fix yet.
