No you don't both need to be new. The purpose of recruit a friend is for an existing player to get his/her friends to play. Therefore, an existing player sends a recruit of friend request to a brand new player. Their accounts are linked in a way where if you level characters together you receive an exp boost. The friend who recruited you also gets bonuses such as a mount I believe.
