There is no significant difference.
"If you want to climb" refers to the rank list, which resets on every ladder.
So gameplay-wise, nonladder and ladder is exactly the same. There are no ladder only items etc.

i think you find better items on ladder, and betterer items on hardcore. bestest items fall in ladder hardcore

Monsters in Ladder have more HP in Ladder than Classic.

Also... allegedly monster AI is improved for Ladder only.
