I generally in the past stick to around this as iv always found it gives me a good drop ratio overall.

however for some reason even after doing over 500-1000 runs. not 1 worth while item has been looted?

have they changed something in game mf wise?

No sir, they have not. It is, and has always been, completely random :).

You're doing it wrong. You must be doing quest bugged boss runs.

I did the act 2 exit thing.

I didn't know you could quest bug duriel. I meant having two characters, making a public passworded game, exiting, then rejoining with another character, then killing the boss. That way you can quest bug any boss. Because since reset december 2018, public games are permanent for 5 minutes without having to wait. Idk if this is for d2r though.

I just run solo baal or D keys and maybe alittle diablo, rest the stuff is not worth running.
