That's the correct recipe. Are you sure that you put a magic ring inside the cube, and not the crafted (or a rare) one?


edit: Make sure not to use a random magic ring. Use those with an ilvl of 71 or higher, cause then you will always get 4 affixes on the crafted item.
You can gamble them on any char that is at least level 76, or you find them in areas that have a level of at least 71
