Fell asleep with my lvl 56 paladin sitting idle in town in A1 hell difficulty with full (as far as I know) health. I came back this morning with my health at 110/919. Walked away again and came back after a few minutes it was down to 94.

lifetap wand on weapon switch

inventory is completely empty

Any ideas why my health is draining? What am I missing?

Are you sure you have black equipped and not malice?

Malice has negative hp regen on it

Malice runeword has -5 drain life, which means losing 1 hit point about every 2 seconds. So you couldn't have slept for very long or he should be at 1 hit point ^^ 100 hit points = 120 seconds, 800 hit points = 960 seconds, which is about 16 minutes.

I removed all gear and the hp drain appears to have stopped. Currently putting on one piece at a time to see when it starts back up.

If you’re on d2r and have prayer on a merc it’s possible for the game to think your prayer level is negative and drain hp.

Removing your gear shouldn’t have changed that though and it’s not something you can really do while afk

these new D2R bugs literally drain your life while your afk and delete items from your inventory
