A guy in game said it’s an old bug

It can drop as early as normal Tombs. It only has a quality level of 26.

The drop system in diablo 2 is something of a mystery. All items has so many parameters to drop, some are

- Item level
- Quality level
- Magic level
- Area level
- Monster level (from which monster to drop)
- Treasure class

And surely other things i don't know.

Basically, i think i read that every single monster has a chance to drop a certain range of items. And yes, tal amu can drop quite early in the game for this reason.

Tombs have many treasures

also once got a sham drop from hell forge dude body .
