server-side problem. you have to be patient and wait. happens if the server doesnt properly recognize a logout process of your char and will be fixed every 1-2 days when the information is reset on the server

at least thats what the support told me some time ago when i had the problem in D2L

Have you tried joining a game with a different character?
Tried joining a different region?

I wouldn't be surprised that with a bug like this you'd be able to dupe items by joining in a different region.

I did all of the above.. I unplugged router overnight and it worked. IDK if that fixed it or if Blizz fixed it. ( I had an active support ticket )
